Paraneoplastic pemphigus: report of a cas
We present a 60-year-old patient with a three-year history of non-Hodgkin lymphoma. At admittance, laboratory test results were normal, but soon leukocystosis with lymphocytic predominance occured, indicating relapse of lymphoma. Immunotyping of a bone narrow specimen confirmed lymphotic leukaemia of B type, kappa +0. High dose corticosteroid treatment combined with azathioprine resulted in a moderate effect. Intravenous human immunoglobulin (IVIG) 20 g over a period of 5 days, in a total dose of 100 g led to rapid healing of skin and mucosal erosions. During treatment the patient developed psycho-organic syndrome and was transfered to another hospital. The patiernt's neglecting to immunosuppressive therapy caused worsening of the disease of the disease and lethal outcome a few months later. Paraneoplastic pemphigus remains a diagnostic and even more therapeutic challenge because of its poor outcome in the majority of the cases.
