Apo E gene rather than ACE gene determines blood pressure values at the early stage of essential hypertension

The role of angiotensin-converting-enzyme (ACE) gene and apolipoprotein (APO) E gene polymorphisms and their association with essential hypertension (EH), the most potent independent factor of cardiovascular (CV) morbidity, have not been clarified yet. The polymorphisms of these two genes were determined in 51 patients (29 male, 22 female ; mean age 38.6+10.5) with mild form of essential hypertension (EH). Blood pressure (BP) was measured in clinical setting with sphygmomanometer (RR) and with ambulatory blood pressure monitor (ABPM). We did not observe any differences in BP or in other clinical and laboratory parameters depending on the ACE genotype (p>0.05). Significant differences were observed between patients with e4 allele of the APO E gene and others in systolic BP (SBP) (167+16 vs. 152+15 ; p=0.02), and pulse pressure (PP)(62+15 vs. 50+10 ; p=0.014) determined with RR. APO E gene polymorphism correlated significantly with SBP (R=0.45 ; p=0.0029), PP (R=0.50 ; p=0.0009) and mean AP values (R=0.35 ; p=0.027) as determined with RR as well as with ABPM (p<0.05). ACE gene polymorphism does not determine clinical characteristics of the early stage of EH. The observed significant correlation of e4 allele of the APO E gene and SBP and PP values is consistent with the recent opinion on the importance of SBP and PP for CV morbidity and mortality.
