Papillary thyroid cancer in a 3.6-year old girl - the youngest case registered in Croatia
The incidence rate of papillary thyroid cancer (PTC) in the first two decades of life is 1 new case per million. In several large series published recently we did not find any case of PTC under the age of four. According to Croatian Cancer Register from 1967 to 2001 a total of 5148 thyroid cancers were noted. Out of them there were 66 patients aged 15-19 years (1.3 %), 29 aged 10-14 years (0.5 %), 5 aged 5-9 years (0.9%) but none younger than four. We present a girl with PTC at the age of three years and a half. In June 2002 enlarged left lateral nodes appeared. In November she was referred to the pediatric endocrinologist. Ultrasound and fine needle aspiration biopsy revealed PTC invading both thyroid lobes with bilateral paraglandular and lateral neck positive nodes. In December we performed bilateral functional neck and paraglandular dissection and total thyroidectomy. We found 11 positive nodes in groups I-V on the right side, and 10 positive nodes in the same groups on the left. The metastases in the group VI were firmly adherrent to the grossly invaded gland. With sharp dissection we devided both recurrent laryngeal nerves from tumor mass and saved them. Postoperative course was uneventful. The girl received 27 mCi of radioiodine therapy. On the last check up there was no evidence of the disease. Our surgical strategy for differentiated thyroid tumors established in the early seventies has not been changed until today. Our basic procedure for all suspicious nodes includes total lobectomy and total thyroidectomy for histologically confirmed malignancies. We strongly advocate functional neck dissection versus selective lateral dissection which does not encompass supraclavicular chain not infrequently invaded by tumor.
