Neuroendoscopic treatment of symptomatic septum pellucidum cyst
Septum pellucidum cysts are more often seen as an accidental radiological finding, but symptomatic cysts are rare. Authors present our experiance in treating symptomatic septum pellucidum cysts using endoscopic fenestration. Authors describe four patients with symptomatic septum pellucidum cysts. All patients presented with pregressive intense headache, acssociated with vomiting in two patients, and behavioral mental changes in two patients. One of them had sequelae of epileptic seizure. In three patients was performed endoscopic fenestration through frontal approach. One patient was operated on via occipital approach. In all patients we performed unilateral cyst fenestration, and in three patients, additionally, was performed fenestration of the cyst into third ventricle and third ventriculostomy. In the patient operated on using occipital approach we performed fenestration of the cyst into both lateral ventricles, so called two-window technique. Endoscopic fenestration is a preferable treatment for symptomatic septum pellucidum cyst. Minimal invasiveness of the procedure allows patients fast recovery and return to every day activities.
