Treatment of severely burned children with cultured keratinocyte grafts
Two girls 1, 5 and 2 years old with extensive burns involving the extremities and the entire back, respectively, were treated with autologus keratinocyte grafts cultured in vitro. The grafts were produced from skin biopsies sized approximately 3 – 4 cm2. Dissolved epithelial cells were cultured as usual on a 3T3 fibroblast layer in appropriate nutrient medium. After three passages, confluent keratinocyte layers were mounted on vaseline gauze, frozen in liquid nitrogen (for the purpose of transportation) and applied on wound beds free of necrotic tissue and infection. The size of the grafts varied from 7 x 10 cm to 10 x 15 cm, and their total areas were 800 cm2 for the first girl and 1200 cm2 for the second one. The mounting tissue was peeled off after four days, leaving small areas and islets of thin epithelium. These foci subsequently coalesced, covering larger areas of the burns. Further recovery was uneventful.
