DISEASE COURSE AND TREATMENT OF PATIENTS WITH COMMUNITY ACQUIRED PNEUMONIA IN ICU

During the period from 1.1.2003. untill 1.9.2004. we collected data from 28 patients treated in an ICU setting for severe communitiy acquired pneumonia (CAP). Diagnosis of severe CAP was made on the basis of the guidelines provided by British Thorasic Society. All patients recieved the same initial antibiotic treatment with either ceftriaxone or penicilin G for 7-14 days combined with azythromicin for 7 days, the route of admission was intravenous and the dosage was previously determined. Laboratory work up and the follow ups were also previously determined, but were also guided with regard to patient's clinical condition. Mechanical ventilation was necessary in 13 patients and 6 of those died. All the patients with lethal outcome had at least one major chronic illnes and three of them were over 60 years of age. Other seven patients who were mechanicaly ventilated and survived needed adjustment of antibiotic therapy, five of them because of hospital infection and two of them because the effect of therapy was absent. In three other patients initial therapy was also inadequate: one had Q-fever, one had S.aureus infection, and in one patient the cause eluded us. The reminder of eleven patients responded well to the initial treatment.
