The phylosophy of Endoscopic Sinus Surgery (ESS) has been adopted world-wide. Nowadays to perform ESS is a matter of prestige and fashion, and serves as an evidence that a certain ENT institution works up to date and therefore is reliable. But, on the other side, this produces the abuse of this method which, of course, is not at all almighty one. No surgical method is almighty. The crucial medical question „ why“ has been changed to „ how“ . Therefore, contrary to the growth of the number of ESS procedures, there is a general decrease of the awareness concerning the properly determined indications, and, in particular, contraindications for the use of this technique. Hence, for less experienced rhinosurgeons it would be of a paramount importance to have an oportunity to contact some of well known experts in the field. Beyond any doubt, the best way is to have a well established telemedicine service, which should be user-friendly and easy available all the time. To be a part of a telemedicine network means the possibility of the mutual transfer of the image in the real time, both during the diagnostical or surgical strategy-making procedures. The most difficult option of this system certainly are the consultations during the surgical procedure itself. To lead someone's hand on the remote-control basis is difficult, although still possible job. The most prominent problem is the question of responsibility: who is guilty in case the things go wrong? Surgeon, a remote expert or both of them? According to the CPME guidelines for telemedicine, accepted by our team, the doctor asking expert's advice remains responsible for treatment and other decisions given to the patient. That is why all physicians practicing telemedicine have to be authorised to practice medicine/surgery in the country or state in which they are located, and must be competent in the field of medicine in which they are practicing it, i.e. in the field of otorhinolaryngology. The principles of medical ethics which are binding on the profession shall also be followed in the practice of our telemedicine. Definitely, telemedicine has the future, providing that all the traps that can occur during its development will be successfully overpassed.
