TNF-a and IL-1a production by diseased human corneas

Purpose. Purpose of this study was: a) to determine pro-inflammatory cytokine secretion from cultured human corneas with different pathology, and b) to establish whether cytokine profile at the time of surgery influenced corneal graft outcome. Methods. Proinflammatory (TNF-a&#61472; and IL-1a&#61472; ) cytokine secretion was measured after 24 hour cultivation of diseased human corneas. 47 corneas were collected from corneal graft receipients suffering from different corneal diseases. Non-inflammatory corneal diseases included: keratoconus (n=8), keratoglobus (n=2), bullous keratopathy (n=11), and stromal dystrophy (n=2). Inflammatory diseases included vascularised corneal scars (n=14), graft rejection (n=6) and corneal ulcer (n=4). Corneas were cultivated at 37oC for 24 hours and frozen at -21oC till cytokine detection. TNF-a&#61472; and IL-1a levels were measured by immunoassay (R&D Systems, USA). Corneas unsuitable for transplantation due to low endothelial cell count (obtained from the eye bank) were used as controls. Statistical analysis was made by Student t-test. Results. The concentration of IL-1a&#61472; secreted from corneas with non-inflammatory diseases was 5.56 &plusmn ; 4.03 pg/mm3. In inflammatory corneal diseases, the concentration of IL-1a&#61472; was significantly higher (11.4 &plusmn ; 5.58 pg/mm3, p<0.05). Low level of TNF-a&#61472; was detected only in case of vascularised leucoma, bullous keratopathy and graft rejection (1.85 &plusmn ; 1.82 pg/mm3, 0.38 &plusmn ; 0.7 pg/mm3 and 0.56 &plusmn ; 0.81 pg/mm3 respectively). Rejection rate in inflammatory group was higher as compared to non-inflammatory one. Conclusion. Increased production of IL-1a&#61472; by corneal recipients with inflammatory diseases suggests its role in corneal graft rejection in humans, similar as it was already found in animal model.
