Total arterial revascularization in diabetic versus non-diabetic patients

Aim: To evaluate that total arterial revascularization is good enough and safe choice in coronary patients with diabetes when compared with non-diabetic patients in one-month follow up Methods: Prospective study of fully computerized data of 463 patients admitted consecutively to total arterial revascularization over a 6-year period (1997-2003) 383 patients were non-diabetic and 80 patients were diabetics ; age and gender were analyzed as demographics factors ; following factors were analysed: arterial hypertension (systolic pressure >140 mmHg), hyperlipoproteinemia (cholesterol>5, 5 mmol/L, LDL>3 mmol/L, LDL/HDL>2) preoperative risk factors: number of diseased vessels (one vessel, two vessel, triple vessel disease), left main stenosis, unstable angina, LVEF, previous myocardial infarction, previous cardiothoracic surgery ; arterial graft distribution: radial arteries and IMA ; complications: postoperative bleeding, perioperative myocardial infarction, superficial and deep wound infection, sternal dehiscence, perioperative transient or permanent neurological dysfunction ; p value was determined by x2 test Results: diabetic patients were significantly older (61 &plusmn ; 7 ; 57 &plusmn ; 9 ; p= 0, 002) diabetic patients had significantly frequent triple vessel disease (50% 37% p=0, 028), significantly frequent hyperlipoproteinemia (55% 43% p=0, 046) there was not found any statisticaly significant difference in one-month period in postoperative mortality and morbidity between two groups Conclusion: Total arterial revascularization is reliable and safe choice both in diabetic and non-diabetic patient ; final conclusions can be made only after analysis of the long term (five or ten years) postoperative results
