Lung Complications Compared to Other Late Complications in Type 1 Diabetes: Is One Predicted by Others?
Lung complications have been described in type 1 diabetes. The aim of the study was to investigate the correlation between late complications, including lung functional tests abnormalities, as well as predictive factors for their development. Forty-eight type 1 diabetic patients, without overt lung disease and with no history of smoking, aged 21-62 years, with diabetes duration of 3-32 years (20 men and 28 women) were included in the study. Correlations between late complications, and significance of predictive factors: patient's age, diabetes duration, systolic and diastolic blood pressure, HbA1c, lipid values, protein excretion rate (PER) and creatinine clearance for their development were investigated. Significant difference was found between lung parameters: carbon monoxide corrected by alveolar volume (DLCO/VA), forced expiratory flow (FEF50) and peak expiratory flow (PEF) in the groups of patients with and without retinopathy (p=0.003 vs. p=0.013 vs. p=0.038). Diabetic neuropathy was determined in 9 of 21 (42.8%) patients with normal DLCO/VA and in 10 of 18 (55.5%) patients without retinopathy, but in all patients with lower DLCO/VA, as well as with retinopathy. Significant correlation was found between retinopathy stages and systolic blood pressure (R=0.45, p<0.05), diabetes duration (R=0.56, p<0.001), HbA1c (R=0.43, p<0.01), PER (R=0.55, p<0.001), and creatinine clearance (R=-0.47, p<0.01), and between neuropathy stages and diabetes duration (R=0.58, p<0.001), HbA1c (R=0.48, p<0.005), PER (R=0.55, p<0.001), DLCO/VA (R=-0.47, p<0.005), and PEF (R=-0.41, p<0.005). DLCO/VA was significantly correlated only with PER (R=-0.68, p<0.001) and creatinine clearance (R=0.54, p<0.001).
