Lipoleiomyoma of the breast parenchyma: a case report.
Breast leiomyoma is an exceedingly rare benign mesenchymal tumor that accounts for less than 1% of all breast neoplasms, and its presence in the mammary parenchyma is the rarest location with fewer than 30 cases reported so far in literature. The differential diagnosis is not simple because of clinical and radiological similarity between the different benign lesions. In this report we present a 40- yearold women with a painless mass in the upper medial quadrant of her left breast. Sonographic images of the left breast showed a hyperechoic homogenous oval mass measuring 54 x 18 mm. Surgical extirpation was performed. The gross examination revealed an oval, well circumscribed, grey to whitish mass measuring 5, 5 x 4, 5 x 3 cm with a whorled cut surface while the microscopic examination of the lesion showed that it was composed of fascicles of elongated smooth muscle cells mixed with mature adipocytes with very low mitotic activity and without atypia or necrosis. Immunohistochemical analysis showed intense staining to SMA, desmin and calponin. Based on the results, lipoleiomyoma of the breast parenchyma was diagnosed. The patient was initially followed for three years before she underwent surgery.
