ASCVD score in metabolically healthy obese patients
Background: Although all obese patients according to the WHO classification have a high risk for health, the latest research shows that this is not always the case. Objective: The aim of this study is to show whether metabolically healthy obese (MHO) people have a lower risk for atherosclerotic cardiovascular disease (ASCVD) compared to metabolically unhealthy obese people (MUO) according to several different criteria. Methods: This cross-sectional study included 169 obese patients (average age 44 years ; BMI 42.38kg/m2) treated at the Clinic for Endocrinology of University Hospital Center Zagreb. MHO is defined using four separate criteria (general, Meigs, Karelis, Angulinar- Salin) including values of fasting glucose, cholesterol, waist circumference, blood pressure, and HOMA index. Atherosclerotic cardiovascular risk was calculated using the ASCVD score. Mann- Whitney U test and ordinary descriptive statistics were used in Statistica v.12. Results: A significantly lower ASCVD risk was found in all MHO individuals depending on a criterion: 1) General criterion: 46 patients had MHO with ASCVD risk median 2.0 [1.3-3.6] vs. 5.1 [1.8-9.5] in MUO (p0.001). 2) Meigs criterion: 57 patients had MHO with ASCVD risk median 2.25 [1.0- 6.8] vs. 6.8 [3.4-9.8] in MUO (p0.001). 3) Karelis criterion: 24 patients had MHO with a tendency towards a lower ASCVD risk (median 1.6 [1.3-4.5] vs. 4.7 [1.9-8.5] in MUO ; p=0.098). 4) Angulinar-Salin criterion: 76 patients had MHO with ASCVD risk median 1.8 [1- 3.6] vs. 6.7 [2.4-10.6] (p0.001). All 46 MHO individuals had low ASCVD risk, while in the MUO group 77 patients had low risk (62.6%), 39 intermediate risk (31.7%), and 7 (5.7%) high ASCVD risk. Conclusion: All MHO individuals had low ASCVD risk, whereby the total risk according to three of four evaluated criteria was significantly lower than in MUO individuals. Studies on a larger number of subjects are needed to confirm our findings.
