Influence of chronic stress on steroid therapy in patients with sudden sensorineural hearing loss
Sudden sensorineural hearing loss is defined as idiopathic hearing loss greater than 30dB on three contiguous frequencies lasting for more than three days. Although there are many theories the real cause is still unknown. Many regiments of therapy are used with controversial success rate. Steroids are mostly used therapy, but the success of this therapy vary a lot. In this research we wanted to investigate connection between level of chronic stress and the success of steroid therapy. Material & Methods In this prospective study we included 85 patients with sudden sensorineural hearing loss treated with systemic steroids. All the patients were asked to fill score symptoms in a Measure of Perceived Stress questionnaire. Pure tone audiogram was measured before the therapy and one month after therapy. Results 49 (58%) of patients had significant improvement in their pure-tone audiogram hearing threshold average after steroid treatment. The difference between pretreatment and posttreatment hearing levels was significantly higher in the cluster with moderate stress compared to clusters with mild and high stress levels. There were no significant differences in hearing threshold recovery after steroid therapy between groups of patients with mild and severe stress. Conclusions Patients with moderate stress could expect better response to therapy than the patients with mild or severe stress. Measure of Perceived Stress questionnaire could help in better selection of patients for steroid therapy but further research is needed to investigate influence of stress level on success of different regiments of therapy.
