Synchronous carotid and cardiac operations
Introduction: For patients undergoing cardiac surgery who have significant carotid stenosis literature supports carotid interventions. Type of management varies between institutions. Aim: We reviewed our results of carotid endarterectomy and cardiac surgery done as a synchronous procedure. Methods: Between January 2008 and September 2021, 91 patients underwent synchronous cardiac operation (either coronary artery bypass (CABG), valve replacement or both) and carotid endarterectomy (CEA). There were 61 males and 30 females. Mean age was 68.91 years. Carotid stenosis was >90% in 42 patients and 70-89% in 47 patients. Seventy-six patients had asymptomatic carotid stenosis and 15 had either TIA or stroke. Results: CEA was performed using a Dacron patch in a majority of cases. In only 7 cases eversion TEA was done. Intraoperative shunt was used in 38 patients. Only 6 procedures were done "off- pump ". Perioperative TIA was registered in 4 patients and 6 patient suffered stroke. Local complications (neck hematoma) occurred in 10 patients but only 5 needed additional procedure. Two patients had perioperative myocardial infarction and in 7 patients pneumonia was diagnosed. One patient died postoperatively. Conclusions: Synchronous carotid endarterectomy with cardiac surgery is a safe and effective procedure in patients with significant carotid disease.
