Lateral alveolar ridge augmentation withautogenous tooth roots and staged implant placement–2 years follow up
Background & Aim: The aim of this pilot clinical study was to assessefficacy and predictability of autologous tooth roots for lateral alveo-lar ridge augmentation and two-stage implant placement.Methods: Preoperative CBCT was made for every of 20 patients. Aftersurgical removal of impacted tooth or periodontally compromisedtooth, the crown was decapitated at the cemento- enamel junction andthe exposed pulp was preserved. The separated tooth root wasadapted to match the size and shape of the bone defect area. Graftswere drilled and fixed using titanium osteosynthesis screw. After6 months of healing commercially available titanium implants wereinserted. The primary endpoint was defined as the clinical width(CW) of the alveolar ridge being sufficient for the placement of dentalimplant without the need for a secondary grafting at 6 months aftersurgery. CW was assessed before and after augmentation healingperiod of 6 months using CBCT scans (ΔDEF). Clinical measurementswere recorded atT1(2 months after implant loading) andT2(2yearsafter implant loading): (1) bleeding on probing (BOP), (2) probing depth(PD), (3) mucosal recession (MR) and (4) clinical attachment level (CAL).Results: Pearson correlation was negative, except forΔPD/ΔCALwhere coefficient was 0.83 (p < 0.01). Linear regression failed toreveal significant correlation betweenΔDEF and CAL and PD values(Coef: 0.182, R2=0.033 p=0.78). BOP variable showed no changesbefore and after the treatment (p > 0.05). There were significant dif-ferences between pre-procedure and post-procedure data for vari-ables PD and DEF. Independent samples t-test between donor teethand variablesΔPD, ΔCAL andΔDEF revealed no statistically signifi-cant differences (p > 0.05).Conclusions: Within its limitations, the present clinical study revealedthat autologous tooth roots may serve as an alternative graft to sup-port lateral alveolar ridge augmentation and two-stage implant placement.
