Changes in techniques of RBC antibody detection and incidence of delayed transfusion reactions
New sensitive techniques for RBC antibody detection might result in an increase ofincidence of delayed hemolytic transfusion reactions (DHTRs) and delayed serologictransfusion reactions (DSTRs). Because of bloodgrouping automatisation at ClinicalHospital Centre, Ortho column agglutination test (CAT) replaced the tube test (TT) in1998. In 2000, DiaMed gel test (GT) replaced the TT in crossmatching, too. DHTR andDSTR data from 1998 to 1999 and data from 2000 to 2002 were reviewed. These datawere compared with our historical data (1990–1997) when TT was used. Withintroduction of micromethods in pretransfusion testing, we observed evident increaseof total incidence of both DHTRs and DSTRs, from 1:4347 to 1:2000 (P< 0.001). Theincidence of DHTR was almost the same, but the incidence of DSTR increased from1:8333 to 1:2500 (P< 0.000001) units transfused. The introduction of GT incrossmatching resulted in another increase of total incidence of DHTR/DSTRs, from1:4167 to 1:1389 (P< 0.001) compared with period when CAT was used only onbloodgrouping. The incidence of DHTR did not significantly change but the incidenceof DSTR increased from 1:5556 to 1:1754 (P< 0.001). 122 RBC antibodies involved inappearance of DHTR/DSTRs, represent 6.9% of totally discovered clinically significantalloantibodies in evaluated periods
