Age and transfusion therapy:autologous blood predonation in primary total hip arthroplasty (pTHA)

Objective:To compare the use of autologous and homologous blood in patientsundergoing pTHA, ages 65 years.Materials and methods:We retrospectively analysed transfusion therapy of 560 ptsunderwent pTHA over last 3 years, divided in two age groups. Group 1: 362 pts<65 years, mean age 52.3 (20–65). Group 2: 198 pts >65 years mean age 70.9 (66–83).Patients from both groups predonated in average 1.6 U (1–2) of autologous blood.Results:Perioperatively received blood transfusion therapy was: (i) autologous blood– 236 patients, 1.55 U/pt (group 1), 135 patients, 1.60 U/pt (group 2) ; (ii) autolo-gous + homologous blood 111 patients, 3.53 U/pt (group 1), 61 patients, 3.98 U/pt(group 2) ; (iii) total 347 patients, 2.18 U/patient (group 1), 194 patients, 2.38 U/pt(group 2), mean preoperative and postoperative hematocrit (Ht) values (%) were for thegroup 1, respectively, 35.3 and 30.1%, and in group 2, 34.8 and 29.6%. Preoperativeand postoperative hemoglobin (Hb) values (g/l) were for the group 1, respectively, 117.9 and 100.2 and in group 2 116.1 and 99.2. No significant differences betweenthese two groups were noted.Conclusion:This study did not reveal any significant difference in transfusion ther-apy, as well as in preoperative or postoperative mean Ht and mean Hb between par- ticipating patients younger and older than 65 years. This finding suggests that olderpatients can successfully participate in preoperative autologous blood donation
