Serologic infection testing of autologous blood donors

Background:Inadvertent homologous transfusion of autologous bloodmay cause serious blood-borne infections in the recipient, dependingof donor status. Aims:We analysed the incidence of blood- borne infections in autolo-gous blood donors. Methods:We tested 2843 autologous blood donors for HbsAg, anti-HCV, anti-HIV and antibodies to Treponema pallidum (TP) between 2000-2005. Standard of confirmation tests were performed for all reactive tests. Results:Total of 60 (2, 11%) autologous blood donors were initiallyreactive for at least one of the markers. Confirmed tests were positivein 55/2843 (1, 93%) patients: HbsAg in 21 (0, 74%) cases, anti-HCV 22(0, 77%) cases, and anti-TP 12 (0, 42%) cases, but no case of positiveanti-HIV was found. Initially reactive, but later confirmed as negative, were one anti-HIV and 4 anti-TP positive cases. The overall frequen- cy of confirmed HBsAg, anti-HCV, anti-HIV and anti-TP was 1, 93 %in all autologous donors. Nine of 22 (40, 9%) anti-HCV positivepatients was transfused with homologous blood before 1993, whenanti-HCV testing become obligated in Croatia, and 14/22 (63, 6%)patients with positive anti-HCV was older than 65 years. Conclusions:Our data show that procedures to minimise the risk ofinadvertent homologous transfusion of autologous blood are neces- sary in spite of the implementation of screening tests in autologousdonation programs.
