Metastases of inguinal region after surgical treatment of adenocarcinoma of the rectum and abdominal wall metastases: a case report

Six years ago 57-old patient was surgically treated at our Department of surgery for adenocarcinoma of the rectum when the rectal resection was performed by the Dixon method. Five years later the process of tumor in the abdominal wall (infraumbilical region), size 7x5 cm was verified with the patient, and the enterirely resection was done with abdominal wall plastic. Pathohistological findings spoke in favor of metastatic adenocarcinoma with no infiltration of muscle tissue around the tumor. There were no metastatic changes on the liver. The patient was admitted at our Department of surgery because of the planned surgical treatment of clinically and citopunctional verified metastatic changes in the right inguinal region. After the presurgery preparation, the patient was surgically treated where intraoperationally the tumor formation of the right inguinal region was found, whitish color at the intersection, with the diameter of approximately 6 cm, above the safenofemoral confluence with fascia drawing. Extirpation of tumor formation with dissection of the lymph tissue of the right inguinal region, together with part of the fascia was done. Exposed vascular structures were covered by local muscle flap. With done histopathologic analysis of stated tissue we have obtained the results of metastatic adenocarcinoma of the rectum, with orderly structured lymph nodes, without any tumor. Postoperative course went orderly with the release at home treatment on the sixth postoperative day, with the recommendation of resting and the continue of the oncological treatment. Surgical examinations control were conducted regulary. Interesting thing about this case review is based on the fact that adenocarcinoma of the rectum is almost never metastasizes in the inguinal area, but the inguinal region is metastasized by anus cancer. Therefore, it is assumed that the metastases in the inguinal region is originate from the abdominal wall tumor.
