Postuveitic macular edema
A 29-year-old woman presented with acute visual loss on both eyes. Ophthalmologic examination revealed intermediate uveitis bilaterally and after systemic workup oral corticosteroids were started as well as local corticosteroid, cycloplegic and non steroid anti-inflammatory drops. HLA tissue typing showed increased relative risk for developing uveitis, reactive arthritis and subacute thyroiditis. The initial optical tomograms of the macula were normal. Combined therapy lead to improvement in her clinical status after month and a half with gradual tapering of systemic corticosteroids. Her visual acuity returned to normal (1.0 both eyes on Snellen charts) and vitreous inflammation diminished. After initial improvement she presented again with loss of vision on both eyes and optical coherence tomography revealed bilateral macular edema which did not improve after periocular corticosteroid injections. She received intravitreal application of triamcinolone acetonide in both eyes and regained normal visual acuity as well as normal appearance of the macular region documented on optical coherence tomograms. The patient remains stabile for the past six months and further follow up is anticipated.
