Teeth whitening
The basic natural colour of the teeth can be changed by discolourations. Various discolourations are indications for whitening. Since not all patients are candidates for this procedure, it is important to determine the causes of pigmentation because it will determine the whitening procedure. The causes of staining can vary and depend on the aetiology, incidence, location, severity and propensity of the chromogen in relation to hard dental structures. Tooth discolouration can be divided into intrinsic (endogenous) and extrinsic (exogenous) or combined, depending on their location and aetiology. Teeth whitening is a procedure of aesthetic correction of tooth colour. Materials used for bleaching contain one of the derivatives of hydrogen peroxide (hydrogen peroxide, urea peroxide or sodium perborate) as an active bleaching agent. It can be applied directly or it is formed in chemical reaction of urea peroxide or sodium perborate. The mechanism of action of hydrogen peroxide is not complete clarified, but bleaching is thought to cause an oxidation process in which large pigmented molecules decompose into less, smaller molecules. Since teeth whitening is a common procedure, the possible negative effects of hydrogen peroxide are a concern. With increased popularity of different types of whitening techniques (in the office or at home) and the accompanying development of countless commercially available products errors associated mainly with the effect of bleach on oral tissues can occur. The most common consequences of these errors are postoperative hypersensitivity and gingival irritations, which are usually mild to moderate and transient. A possible side effect during whitening of avital tooth is external root resorption, although it is also successfully prevented. No permanent changes in hard dental tissues were observed. Not all patients are candidates for the whitening procedure ; therefore, it is most important to perform a thorough clinical examination and determine the presence of indications and contraindication for tooth bleaching.
