Correct process of history taking and reaching the accurate diagnosis in restorative dental medicine with appropriate choice of treatment is a precondition or each simple or complex intervention. It is part of treatment plan and requires understanding of basic steps in diagnostic workup. Patient history, or medical history, or anamnesis, includes interviewing the patient to collect health-related information. Specially designed history-taking cards may be helpful. Additionally, to reach the final diagnosis, a clinical examination should be performed, including extraoral and intraoral examination using clinical tests: inspection, palpation, percussion, tooth mobility, periodontal probing, transillumination, pulp vitality tests (thermal, electrical), bite test, anaesthesia test, and cavity drill test. In addition to clinical examination, diagnosis, and assessment of future treatment, the existing fillings should be evaluated for their structure, integrity, fissures, anatomical shape, periodontal interface, occlusal and aproximal contact, and aesthetic aspect. In this chapter, we will describe the developmental tooth anomalies including abnormalities in teeth number, size, shape, structure and colour. In addition to lesions caused by cavities, unrelated sclerotic lesions of hard dental tissue are also common and serious problem in dental practice, not only for their prevalence, but also for their aetiology, diagnosis, and restorative treatment choice. Due to their complex aetiology and common effects of different and long-term wearing away mechanisms, individual processes of wearing away of hard dental tissue are difficult to discern. For clinical success, a time diagnosis of causative mechanism is of utmost importance in order to predict further development given the planned treatment during oral rehabilitation.
