Chemical changes of dentin after different activated irrigation protocols
Objectives: This pilot study aimed to evaluate the chemical changes in intracanal dentine after exposure to sodium hypochlorite (NaOCl) and ethylenediaminotetraacetic acid (EDTA) in combination with passive ultrasonic irrigation (PUI) and novel Er:YAG shock wave enhanced emission photoacoustic streaming (SWEEPS) when assessed using Fourier transform infrared spectroscopy with total attenuated reflectance (FTIR-ATR). Methods: The root canals of 28 extracted human premolars were instrumented with ProTaper Next technique up to size 25, .06 in combination with distilled water and distributed randomly into groups according to the irrigation protocol used (n=4): (Control group) distilled water (conventional syringe irrigation, CI), (1) 3% NaOCl (CI, 30s), (2) 15% EDTA (CI, 60s), (3) NaOCl (30s) + EDTA (60s) (CI), (4) NaOCl (30s)+ EDTA (60s) + NaOCl (30s). Groups 4. was further divided into three subgroups according to the activation technique used: CI, PUI, SWEEPS. After the irrigation protocol, the sample of intracanal dentin was extracted using sterile ProTaper Next instrument size 40, .06 and rinsed with miliQ water. The dentin samples were analysed using FTIR-ATR in order to evaluate the amide I and phosphate absorption bands and calculate their ratio. Due to the small number of samples, the results were analysed by Mann-Whitney U test (p<0.05). Results: Considering the results in medians, FTIR- ATR evaluation showed significant collagen degradation after sequential SWEEPS activation of NaOCl+EDTA+SWEEPS (p=0, 46) and CI of NaOCl+EDTA compared to the control group. PUI and CI of NaOCl+EDTA+NaOCl did not show any significant changes in amid I/phosphate absorption bands (p>0.05). Conclusion: Under the limitations of this study, SWEEPS activation of sequential use of NaOCl and EDTA in final irrigation protocol caused changes in dentinal collagen.
