Transnasal high flow humidified oxygen preoxygenation before emergency tracheotomy in congenital tracheomalacia.

Tracheomalacia is an abnormal collapse of the tracheal walls. It may occur in an isolated lesion or can be found in combination with other lesions that cause compression or damage of the airway. Tracheomalacia is usually benign, with symptoms due to airway obstruction. Tracheomalacia may occur as a primary lesion, in which case the cartilage of the trachea develops abnormally. This results in tracheal walls that are soft and collapse during respiration.We want to present a case report of a 2 month old infant with severe respiratory distress due to congenital tracheomalacia. Surgical tracheostomy was needed because of loss of compensatory mechanisms. The meticulous preoxygenation during airway management is mandatory CONCLUSION: Humidified high flow nasal prong (cannula) therapy is a method of providing continuous positive airway pressure (CPAP) to children requiring respiratory support or in respiratory distress. Although widespread applications in many hospitals in pediatric wards and ICUs and PICu. there is a ongoing applicability of HFNF in the ORs for preoxygenation especially in threatened airways, thus extending apnea time without desaturation.
