Influence of perfectionism on patient's decision to accept orthognathic surgery

Aim: Some patients tolerate well their severely pronounced malocclusions and do not deem correction necessary while others with minor malocclusions seek orthodontic evaluation and accept treatment, suggesting that personality traits may influence patient’s perception of dentofacial deformity. The aim of the study was to investigate the influence of dimensions of perfectionism on patient’s decision to accept surgery. Materials and methods: In a convenient sample of subjects (N=108) (68% female) aged 14–53 years (median 18, interquartile range 17–26), with skeletal malocclusions of Index of Orthognatic Functional Treatment Need grades 3-5 (moderate to very great need for surgery) levels of perfectionism were compared between subjects who accepted orthodontic preparation for orthognathic surgery and those who refused (N=55 vs. 53). Fisher and Mann-Whitney test were used to analyse differences. Results: Subjects with skeletal malocclusion who accepted orthognathic surgery were older, and exhibited higher level of global perfectionism dominantly in concern over mistakes and personal standards dimensions with small to moderate effect size (p≤0.040 ; r=0.211-0.337). Males accepted surgery somewhat more often than females (57 vs. 48%), but the difference was not statistically significant. Conclusion: Higher level of perfectionism with both adaptive (high personal standards) and the maladaptive dimensions can make the background of the patient’s perception of dentofacial deformity and thus the decision to accept orthognathic surgery.
