Universal newborn hearing screening (UNHS) eas accepted wide-spread as a prerequisite of an early diagnostic of congenital hearing impairment. Screening with otoacoustic emission based methods is present in most maternity units under the assumption of an extremely low incidence of central auditory processing disorders. The UNHS eas started for the first time in Croatia on the beginning of February 2002. in one maternity unit in Zagreb with about 3000 newborns per year. Two-staged and &#8220 ; ; bilocal&#8221 ; ; model is used: first stage in maternity unit before discharge and second stage in tertiary-care university hospital with an audiologic department. At first stage an automated transient evoked otoacoustic emissions (TEOAEs) registration is used, at second stage &#8220 ; ; classical&#8221 ; ; TEOAE registration, tympanometry and ENT examination are used. In something more than four month 972 newborns were tested: 34 (3, 5%) of them failed to have TEOAE in one ear and 11 (1, 1%) bilateral. Two of eight babies passed two weeks later the second stage as normal, six needed diagnostic ABR and three of eleven did not refer yet. Two of 972 screened babies were diagnosed as bilateral hearing impaired &#8211 ; ; one as a mild conductive hearing loss due to palate cleft, and one as a profound sensorineural hearing loss due to a combination of four risk factors. Preliminary results show that the model is feasible within the existing health care structure. There is a plan to start a national UNHS program in all 34 Croatian maternity units at the beginning of autumn this year. Two-staged, &#8220 ; ; bilocal&#8221 ; ; and &#8220 ; ; bimodal&#8221 ; ; model is planed: two-staged and &#8220 ; ; bilocal&#8221 ; ; as it was explained before (seven audiological departments in Croatia shall be engaged for second stage), &#8220 ; ; bimodal&#8221 ; ; because the new automated ABR for second stage is planed.
