WHICH QUALITY OF LIFE DIMENSIONS MOSTLY DISCRIMINATE SUBJECTS WITH AND WITHOUT FACIAL SKELETAL DISCREPANCIES AND INFLUENCING DECISION TO ACCEPT SURGERY?

Aim: To investigate differences in quality of life in subjects with and without facial skeletal discrepancies that influence patients' decision to accept orthognathic surgery. Additionally, is there an influence of gender. Materials and methods: Cross-sectional study included 190 Caucasian subjects (65% females), aged 17-46 years. 95 subjects were consecutive patients referred to University Dental Clinic Rijeka for skeletal malocclusion treatment (58 accepted suggested surgery, 37 refused). Other 95 were age and sex matched controls, without malocclusions. Psychosocial and functional issues were evaluated by Orthognathic Quality of Life Questionnaire while complexity of skeletal discrepancy and treatment need were assessed by the Index of Orthognathic Functional Treatment Need. Results: Discriminant analysis demonstrated that three groups differed mostly in facial aesthetic concern (r=0.878), followed by altered oral function (r=0.766), then by social interactions (r=0.668) accounting for a total of 96% variability (p<0.001). Subjects who accepted surgery with the highest aesthetic concern, impaired oral function and disruption with social contacts are clearly distinguished from the control group (without skeletal malocclusions) with the lowest values in these dimensions. Awareness of facial deformity did not discriminate three groups. Similar results were seen when gender was included in the discrimination analysis. The highest values were attributed to women who agreed to surgery, and the least to men in the control groups. Conclusion: Facial aesthetic concern, impaired oral function and social interactions are major issues encouraging subjects with facial skeletal discrepancies to choose orthognathic surgery, especially in females.
