Do sex offenders with child victims suffer from a pedophilic or a personality disorder?

Objective: It is well known that sex offenders with child victims have higher prevalence of parahilias, mostly pedophilic disorder. In addition, it is reported that among such offenders, between 25% and 50% suffer from antisocial personality disorder. This study investigates distribution of these disorders among individuals submitted for a forensic evaluation. Design and Method: During the period of nine years, out of 650 cases referred for a forensic evaluation in University Psychiatric Hospital Vrapče, Croatia, 37 cases pertained to offenses involving a child victim. The average age of the victims was 10.89 (SD=4, 32). All diagnosis were reached according to the ICD-10 and DSM-V criteria. No multiple diagnoses were given to any of the offenders. Proportions of each diagnosis was calculated. Results: Sex offenders with child victis were most frequently diagnosed with specific personality disorder (70, 2%). Naricissistic personality disorder was diagnosed in 35, 1 % cases, whereas antisocial and borderline personality disorder were diagnosed in 32, 4 % and 2, 7% of cases, respectively. On the other hand, every third offender was diagnosed with either paraphilic disorder (16, 2%) or pedophilic disorder (13, 5%). Conclusions: In accordance with the up to date literature, our study also confirmed that sex offenders with child victims more often suffers from personality disorder, especially of narcissistic and antisocial type rather than from pedophilic disorder. Moreover, diagnosis of paraphilia was given equally as frequent as a diagnosis of pedophilic disorder. Reason for this could be failing to meet diagnostic criteria for pedophilic disorder.
