PR022: Cytomegalovirus and Epstein Barr Virus role in etiology and development of periodontal disease
Background & Aim: Virus etiology of periodontal disease and its development was suggested by few studies through changing the host response to local subgingival microbiota. Our aim was to investigate a possible connection between Cytomegalovirus (CMV) and Epstein Barr Virus (EBV) with periodontal disease in patients with coronary heart disease. Methods: Subjects (n = 60) were recruited from hospitalized patients that had to do coronary angiography due to their primary illness at the University Hospital Centre Zagreb. A sample of peripheral venous blood (10 ml) was collected to perform the antibody titration analysis for CMV and EBV. The centrifuged serum was analyzed by VIDAS® (bioMérieux, http://www.biomerieux-diagnostics.com/) using ELFA method (Enzyme Linked Fluorescent Assay). Full periodontal examination and indices were measured. 52 samples were representative and were included in statistical analysis. Results: The presence of VCA IgM specific antibodies tells us about a recent primary infection with CMV and EBV, while the presence of VCA IgG specific antibodies is a sign of infection somewhere farther in the past. Of all subjects tested for specific antibodies for EBV and CMV, all of them were positive for EBV VCA IgG (100%), and most of them for CMV IgG (90.7%). For all subjects who were seropositive to VCA IgG and negative to VCA IgM, we can conclude that they had similar results: more than 60% of them had some form of periodontal disease, mostly 40% chronic periodontitis and 20% aggressive periodontitis. More than 60% had at least one pocket depth of 6 mm and above. Deeper pockets were significantly more frequent in a group that was VCA IgM negative, p < 0.05. If we look at known risk factors, male sex dominated with 80% and only 25% of subjects were never smokers. Conclusion: Development of periodontal disease might be associated with CMV and EBV infections, more frequently with chronic periodontitis, however further research is required.
