Comparison of dental and periodontal condition of patients unexposed and exposed to metal orthodontic appliance

Aim: To analyze changes of hard and soft oral tissues in patients due to exposure to a fixed metal orthodontic appliance. Methods: Sample consisted of 30 subjects aged 12-17 years (50% female). Dental and periodontal status was assessed before applying fixed orthodontic appliance and one year after it's exposure in oral enviroment. Teeth with white spot lessions of enamel, pH value of dental biofilm, dental biofilm accumulation level and gingivitis extent were measured. Wilcoxon test was used for statistical analysis. Results: Exposure to orthodontic appliance increased the number of teeth with white spot lesions present (from an average of 1.7 to 2.3), dental biofilm accumulation level also increased (from 0.9 to 1.1), as did the gingivitis extent (from 25% of sites to 35%) and the average pH (from 6.1 and 6.4). Changes were not statistically significant, but gingivitis and pH were close to the limit of statistical significance. The gingivitis increased in 63% of subjects, accumulation of dental biofilm in 53%, and white spot lesions in 37%. The acidity of biofilm increased in 40% of cases, while it decreased or remained the same in 60%. The change in pH of the biofilm did not correlate with the change in the number of teeth with white spot lesions, the biofilm accumulation, or gingivitis. Conclusion: A fixed orthodontic appliance creates more retention sites for dental biofilm which increases enamel demineralization and gingivitis. The decrease in the acidity of the dental biofilm could be due to the increased salivary flow rate or effects of oral corrosion products of orthodontic appliance alloys.
