The effect of controlles weight reduction on carotid intima-media thickness in obese patients

Background: Carotid intima-media thickness (CIMT) is a validated clinical tool for identifying initial atherosclerotic changes. CIMT above 0.9 mm can be considered as a possible abnormality, although age and sex specific percentiles of CIMT give more accurate insight into patient’s vascular age. Methods: We conducted a prospective research comparing initial CIMT, Montreal Cognitive Assessment (MoCA) score and body weight with the ones measured after a six and a twelve month periods. Patients were considered obese if their BMI was >30 kg/m2. All patients were divided in three groups (group A, B and C) considering their weight reduction: more or equal to 5% (A), less than 5% (B) or no weight loss (C). The total number of patients was 31 (9 males, 22 female) with a mean age of 52 (22–79) years. CIMT was measured two times at both, common carotid artery (CCA) and carotid bulb (CB). Average at each site was calculated. Results: At initial assessment average weight of our patients was 126 (78.9–202) kg with average MoCA score of 27. After follow up average weight reduction was 10 kg and average MoCA score did not change. At the initial assessment the percentage of patients on 75th or higher percentile was 43 for left CCA and 32 for right CCA. After weight reduction the percentages were 39 for left CCA and 25 for the right one. Conclusions: On follow up examination patients that had some weight reduction showed tendency towards lower median percentile values of CIMT. The difference between CIMT of the left CB at the initial assessment and after follow up was statistically significant between those who lost and those patients that did not lose weight (p<0.05). A larger number of patients is needed to confirm the effect of weight reduction on CIMT.
