Opportunistic premise plumbing pathogens: a new challenge in the hospital

In the last few years much attention has been paid to a group of environmental microorganisms whose source is water from the distribution system in residential buildings and health facilities. The name of these group of bacteria is Oportunistic Premise Plumbing Pathogens (OPPP). The group encompasses various bacteria such as species from the genera Legionella, Mycobacterium, Pseudomonas, Stenotrophomonas, Sphingomonas, Aeromonas and some others. This group of pathogens is colonizing a part of the water distribution system, which includes hot and cold water pipes, and associated devices such as heaters, showers and taps. Their common features include disinfection resistance, adhesion to the tube surfaces, and biofilm formation. Their frequent occurrence is due to the fact that they can survive the standard treatment for drinking water. Water in the hospital distribution system can be a reservoir of potentially pathogenic microorganisms that can lead to the development of healthcare-associated infections. Therefore, we analized bacteria in biofilm on mesh on the faucets in intensive care units and in households. In the hospital setting, antibiotic-resistant bacteria dominated in the biofilm on the mesh, while a mixed biofilm was found on the mesh in the households and no resistant microbes were found. Although hand hygiene is the most important measure in preventing healthcare- associated infections, the formed biofilm at the outlet of water pipes, aerators and sinks can be a source of contamination of healthcare workers hands during hand washing. For now, due to the usual presence of OPPPs and their ability to create biofilm, it is necessary to regularly maintain, clean mesh on faucets or change them more often, especially in high-risk units.
