“Isolated stenosis of infrarenal aorta presented as blue toe syndrome successfully treated with kissing-dilatation with DEB“
45-year-old female smoker presented to the emergency department with a ''Blue toe syndrome’’. Detailed imaging using CTA was performed to precisely assess anatomy of the lesion. Soft eccentric plaque was located in infrarenal aorta, narrowing the aorta by 80% and partially blocking origins of CIA. ABI on both legs were significantly reduced to 0.58 left and 0.54 right. Lesion was predilated and then treated with DCB, using kissing-balloon inflation. Immediate post-procedure angiography of infrarenal aorta and iliacs showed no residual significant stenosis. After two-month follow up, CTA revealed a thin soft plaque with no hemody-namic repercuss- ions, confirmed by value of ABI of 0.94 on left and 0.96 on right. Here we report the first attempt of endovascular treatment of isolated infrarenal stenosis by kissing-dilatation with DCB. Careful patient selection considering her age, comorbidities and distribution of stenosis, together with experienced operator lead to successful early outcome with no adverse effects, as well as two-month follow-up with no complications or signs of recurrence of the disease.
