Pharmacist’s intervention in pharmacotherapy treatment of Helicobacter pylori – a case report

Pharmacists present the last professional supervision in the medication cycle and have a great responsibility in preventing medication errors. Herein, we present a case of a successful pharmacist’s intervention in eradicating Helicobacter pylori (H. pylori) infection - the main cause of gastritis, gastroduodenal ulcer disease, gastric adenocarcinoma and mucosa- associated tissue lymphoma. Eradication of H. pylori is a very sensitive process and requires a thorough adherence to guidelines. A 28 year old female diagnosed with H. pylori infection was assingned by her physician with the following therapy: esomeprazole 40 mg q.d., metronidazole 400 mg b.i.d and ciprofloxacin 500 mg b.i.d. While taking the medication review, the pharmacist found this to be the second eradication protocol as the first treatment protocol consisting of pantoprasole 40 mg q.d., claritromycin 500 mg b.i.d. and amoxicilin 1000 mg b.i.d. failed to give results after seven days of treatment. The pharmacist suggested the following therapy modification to patient's physician based on the guidelines for the treatment of H. Pylori: proton pump inhibitor dose increase (q.d. to b.i.d.) and extending the eradication regimen to 14 days. After completion of the modified eradication protocol the patient was cured. Pharmacist’ intervention contributed to successful H. pylori eradication. Awareness of possible medication errors throughout the medication cycle is crucial as it can improve the therapeutic goal and reduce the cost of treatment.
