Changes of bone volume and height using biphasic β-three-calcium phosphate/hydroxyapatite in sinus grafting – preliminary results
PURPOSE OF THE STUDY: The purpose of this prospective clinical controlled randomized study is to compare the use of two different synthetic bone graft materials during the surgical procedure of maxillary sinus floor augmentation. MATERIALS AND METHOD:68 surgical procedures of sinus grafting have been performed on a total of 43 patients. The patients were subdivided into two groups on the basis of the material used. The material was randomly chosen and applied on each patient. In one group pure β-three-calcium phosphate (100% β - TCP) was used, whereas for the other group a biphasic bone substitute of β-three- calcium phosphate and hydroxyapatite (60% HA, 40% β-TCP) was used. Radiological analysis (CBCT) was performed immediately after sinus lift procedure and 6 months later, prior to implant placement. Radiological analysis supplemented by the use of an appropriate 3D software was performed with measurements of regenerated bone volume and height. RESULTS:There is an evident volume reduction of the augmented area after a 6 month healing period ; 0, 39 ± 0, 24 cm3 (22, 2%) in the pure β- three-calcium phosphate group and 0, 13 ± 0, 17 cm3 (6, 6%) in the biphasic group. This volume reduction is statistically significant (p<0, 001). The height of the augmented area in the pure β- three-calcium phosphate group changed from 13, 13 ± 1, 49 mm to 12, 03 ± 1, 69 mm during the six-month period, which means a decrease of 1, 1 ± 1, 01 mm or 8, 35%. The biphasic group showed a change in height from 13, 97 ± 1, 92 mm to 13, 49 ± 1, 9 mm, which means a decrease of 0, 48 ± 1, 16 mm or 3, 15%. This change in bone height is statistically significant (p<0, 05). CONCLUSION: Bone substitutes used in this study support bone regeneration. It is observed that dimensional changes include volume and height reduction after 6 month healing period. There are some advantages for the biphasic material which shows less volume and height loss of the augmented area.
