Innovative approach in reducing anxiety and pain during dental procedures in children and adolescents

Objective: The aim is to compare different methods in achieving pain control during dental procedure. For the purpose of the study local anesthesia - the standard procedure for achieving analgesia, and TENS device will be compared. Transcutaneous stimulation of the electrical nerve (TENS) is a non- pharmacological method that is widely used by medical and paramedical experts to manage acute and chronic pain. Previous studies have shown that the use of TENS reduces anxiety in pediatric population, so TENS is effectively used to combat pain during various procedures, such as fissure sealing, cavity preparation, minor extraction, and endodontic procedures. Since TENS is a non- invasive method, and no complications are expected, the aim of the study is to prove that it is equally effective or more effective in reducing anxiety and pain in young patients as compared to local anesthesia. Material and methods: The degree of dental anxiety will be measured immediately before and after dental procedures. In the waiting room, the patient will be instructed how to fulfill the „STRES TEST” immediately prior to examination, informative conversation and dental treatment (filling grade I on permanent molars). We will use the Children’s Fear Survey Schedule - Dental Subscale adapted for adults, ASI (Anxiety Sensitivity Index) - 3, Norman Corah Dental questionnaire, Self- Assessment Manikin Scale. Based on the diagnosis of dental anxiety and the selection of patients who need restorative treatment on permanent molars, patients will be arranged in three groups at their first visit: 1) Group A of 40 children without any anesthesia ; 2) Group B with 40 children treated by TENS appliance ; 3) Group D with 40 children with standard local anesthesia will be applied. Results: The results are expected to show that relaxation techniques and the second and third group of respondents will feel much more relaxed ; dental anxiety is expected to decrease ; the patient is expected to have more confidence and should be more motivated to continue dental visits for treatments or regular checkup. Conclusion: The therapist should individually and with a lot of understanding and patience approach to patients and eliminate dental anxiety and phobias while working with them in order to strengthen trust and solve their psychological and physical symptoms.
