Evaluation of 20 years long term posttreatment changes using 3D gnathometric analyses

AIM: Movements of teeth in long term posttreatment period is unpredictable. Eventhough there is improvement in tooth alignment after orthodontic treatment and in posttreatment stage, some research shows there is tendency to return to original malocclusion. The aim of this study is to investigate 20 years long term posttreatment changes in extraction and nonextraction cases. MATERIALS AND METHODS: The sample consisted of 52(41 female and 11male) patients with Class I and II malocclusion, treated with (27 patients) and without extraction (25 patients). Their dental casts were obtained before treatment (T1), after treatment (T2) and at a mean of 17 years long term posttreatment (T3). All patients were treated by one experienced orthodontist. Retention protocol was 3 years ( upper Wraparound retainer and lower bonded retainer). All dental casts were scanned with Ortho Insight 3D Scanner ( Motion View LLC, Hixon, USA) and transfered into STL format. They were analyzed by using custom software which is developed in Matlab programing environment with Little's irregularity index, intercanine width, intermolar width and interdental angle. Interdental angle was measured between 2 lines, each one connecting mesial and distal contact points of a anterior contralateral teeth. Statistical analyses were done using student t test in RStudio (Boston, MA, USA). Research was granted by University of Rijeka (uniri biomed 18- 71-1203). RESULTS: Irregularity index was initially higher in extraction group than in nonextraction group. It was significantly corrected at the end of orthodontic treatment in both groups. It increased at T3, but remained clinically stable, under 3.5mm of displacement .The biggest amount of displacement from T2 to T3 was reported between teeth 22-21 and 43-42. Intercanine width and intermolar width changes were not statistically significant. Extraction cases had smaller interdental angles at T2 than non extraction cases indicating more tapered arch form. Interdental angles increased from T2 to T3, and those changes were more pronounced in extraction cases. However, in non extraction cases, interdental angles on average changed 2.6 degrees, which is not clinically significant. CONCLUSION: Both extraction and non extraction orthodontic treatment showed clinically relevant stability in 20 years posttreatment changes. Eventhough extraction cases showed smaller interdental angles thus more tapered arch form, it seems it didn't affect long term stability.
