Objectives: To evaluate ASCT in ML patients (PTS) by assessing its: 1) toxicity in terms of transplant related mortality (TRM), hematopoietic recovery. 2) efficacy in terms of CR, PR an NR achieved, and long-term efficacy expressed as both disease free (DSF) and overall survival (OS).Patients and Methods: 102 pts with ML including 62 non-Hodgin lymphoma, and 40 Hodgkin lymphoma pts, (55 male, 47 female ; age 17-67, median 34) received BEAM myeloablative treatment followed by ASCT. Four pts were transplanted in CR, all others had active disease at time of transplantation. All pts received heavy prior treatment with a median of 2 lines of chemotherapy (range 1-6), median of 8 chemothrapeutic cycles (range 2-31), and average of 10.6(range 0.8-59.2, SD 11.5)x10 6/kg CD34+ cells was reinfused and G- CSF (5 microgr/kg) was administered during leukopenic period to all but one patient (median and mode of 9 days).Results: median time to WBC recovery (> 1x10 9/L) was 10 days (range 8-20), while platelets recovered ((>20x10 9/L) in median of 11days (range 5-30). A median of 3 febrile days (range 0-20) was observed. Of all patients transplanted 73 achieved CR (73%) and 25 achieved PR (25%). Four pts died during pancytopenic period (TRM 3.9%). At eight years from transplantation projected OS is 58.9%, while DSF is 57.3%. cox multivariate analysis identified that only age (p=0.03), number of infused CD34+ cells (p=0.01), time to WBC recovery over 1.0x10 9/L (p=0.02) were factors that independently entered the model at a significant level for overall survival post ABMT.Conclusions: Given that all pts were heavily pretreated and had active disease at a time of transplantation ASCT toxicity can be considered as acceptable. A high remission rate was achieved (CR+PR=98%). Projected DSF of 57.3% at 8 years in this group of pts is highly satisfactory. Independent predictors of survival were age, number of CD34+ cells reinfused and time to WBC recovery.
