Pathological fractures of the femoral neck
Location of pathologic fracture in proximal femur and femoral neck is unique. This region is susceptible for fracture through benign or malignant bone lesion due to special biomechanical circumstances and weight-bearing. Numerous attempts are made to identify radiographic predictors of lesion with the risk for fracture. Contemporary treatment should be individually tailored to the size, specific location and availability of bone stock for purchase. Biopsy is mandatory in any case, but the timing of biopsy depends on radiographic and magnetic resonance appearance. Currently, treatment comprises precutaneous technique of curettage, mechanical disruption of the cyst walls, bone grafting or substitutes and in the case of unstable fracture fixation with variety of internal devices (flexible intramedullary nailing, screws, angle plate, plate and screws) and external fixation.
