Intramedullary telescoping nails in paediatric orthopaedics
Purpose: The orthopaedic treatment of the weak bone deformities in paedatric age is performed by intramedullary (IM) implants with telescoping mechanism, almost exclusively. Methods: IM telescoping devices are mainstay of surgical treatment to improve walking ability in several pathological conditions such as osteogenesis imperfecta (OI), fibrous dysplasia, hypophosphatemic rickets, Stüve-Wiedemann syndrome and congenital pseudarthrosis of the tibia (CPT). Besides femur and tibia IM telescoping nails could be used in upper extremity e.g. humerus. Results: By reviewing current literature and personal experience IM telescoping nails are safe and effective option to correct and/or prevent deformity in growing patients with rare musculo-skeletal diseases. Telescoping IM nails have survivor life-time of five years according to evidence available. Usually, after this period exchange of IM telescoping nails is necessary, due to natural growth and gain in weight of the patient. Conclusion: IM telescoping nails are useful tool to treat weak and growing long bones in patients of paediatric age.
