Impact of the comprehensive medication management services on the clinical status in patients with hypertension in the Health Centre Zagreb – Centre

Introduction In 2016 cardiovascular diseases were the main cause of death globally, with hypertension as the leading risk factor 1. This study aims to evaluate the clinical impact of Comprehensive Medication Management (CMM) services provided to patients with chronic diseases on blood pressure as the main outcome measure. Materials and methods A prospective, observational study was conducted with patients followed-up for 14 months at the primary care clinic, Health Centre Zagreb – Centre (HCZC). Patients with chronic conditions taking multiple medications were being referred to the pharmacist by their general practitioner (GP). Identified drug therapy problems (DTPs) were classified according to the Pharmacotherapy Workup method proposed by Cipolle et al (2012) 2. Results Overall, 81 patients were included in the study, of which 51 (63%) were female, with a median age (range) of 71 (32 - 87) years. On average, patients used 8 (2 - 19) medications and had 5 (1 - 11) comorbidities. Hypertension was the most frequent medical condition (56.8%). The total number of 229 DTPs were identified with an average of 2.8 DTPs (± 1.7) per patient, of which 21.4% pertained to hypertension. Patients with hypertension and at least two consultations (N=46) were taken into consideration. Out of them, only 28.3% reached positive clinical outcomes at the first consultation. At the last consultation, the percentage of hypertensive patients with improved clinical status was significantly higher (63.0%) (p<0.001). Conclusions This study showed positive impact of CMM services on patients’ blood pressure, thus implying an important role pharmacists have in the care of polymedicated patients with hypertension.
