Drug therapy problems - the clinical domain of the pharmaceutical care

Introduction: A drug therapy problem (DTP) is any undesirable event experienced by a patient that involves, or is suspected to involve, drug therapy, and that interferes with achieving the desired goals of therapy and requires professional judgment to resolve. The pharmacotherapy workup was developed and adopted as a systematic problem-solving process to identify, resolve, and prevent DTPs, establish therapy goals, select interventions and evaluate outcomes. This process is used by a pharmaceutical care practitioner providing Comprehensive Medication Management services. Identified DTPs are the unique contribution and a major responsibility of the pharmaceutical care practitioner, and if not resolved, could have serious clinical consequences 1. Aim: To introduce the categorization of DTPs defined and developed by a research group at the University of Minnesota in 1990 2 ; to identify DTPs trough the demonstration of the clinical case and create a care plan for the patient. Teaching and learning activities: Following a brief introduction to the categorization of the DTPs, participants will work in small groups and will be provided with a brief case history of a patient as to identify and resolve DTPs and create a care plan. Learning outcomes: By the end of the workshop, participants will be able to: • Collect all patient’s data relevant for DTP identification • Resolve the identified DTPs and prioritize them • Discuss the importance of DTP identification for the pharmaceutical care practitioner providing medication therapy management
