Helicobacter pylori resistance to antibiotics in Primorsko-Goranska County
High resistance rates to clarithromycin and metronidazole revealed in H. pylori strains isolated from patients in Primorsko-Goranska County provides important information that triple therapies based on these drugs are not useful as first-line treatment in this region. The regional rates of H. pylori resistance to amoxicillin and tetracycline are very low and stable during the study period which suggests that these antibiotics should be utilized in the initial treatment regimens. Increasing levofloxacin resistance, together with worrying appearance of multidrug resistance found in ten percent of clinical isolates indicate necessity of H. pylori susceptibility testing in order to guide "rescue" treatment in patients who have failed two prior treatment regimens which are typically chosen empirically. Surveillance of local antimicrobial resistance is crucial to accurately evaluate resistance rates for the optimization of both regional and personalized H. pylori treatment strategies
