Emotional recognition in first episode psychosis: a follow- up study
Objectives: The purpose of this study was to investigate emotional recognition in patients during the acute phase of the first episode of psychosis (FEP) and its changes over the course of illness. Methods: We conducted a study involving 50 FEP patients addmitted to Psychiatric University Hospital Vrapce and University Hospital Centre Zagreb and 50 healthy controls (HC). The diagnosis was confirmed using Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (American Psychiatric Association, 2013). Assessment included detailed clinical interview, clinical ratining of psychopatology with Positive & Negative Syndrome Scale (Kay et al., 1977) and two tasks regarding emotional recognition: Penn's Emotional Recognition Task (Gur et al., 2002) and IFEEL Pictures (Emde et al., 1993). Measurements were performed at two time points: baseline assessment for controls and FEP patients during the acute phase of psychosis and after 12- 18 months of follow up when stable remission of FEP was accomplished. Statistical analysis was performed using the χ2, McNemar- Bowker test and bivariate correlations, using the SPSS 17.0 statistical package. Results/ Conslusion: HC statistically significant more correctly recognized fear, happiness and sadness compared to FEP patients in the acute phase of illness, but also during the remission phase, although the differences were found only for neutral and sadness. In the FEP sample, significant correlations between psychopatology and poor recognition of negative emotions (fear, sadness) were found. Therefore, emotional recognition seem to be impaired in the acute phase of the FEP, which seem to decrease, but not disappear, also in the subacute phase, possibly reflecting the remaining negative symptoms in FEP.
