A non-surgical management of internal-external perforation with MTA
The internal-external perforation is a perforation from the pulp space to the periodontium commonly as a sequel of a iatrogenic trauma. It results from overtreatment with endodontic burs or instruments. In this particular clinical case tooth 25 served as an abutment tooth in a bridgework. A perforation was situated on the lateral internal wall of the root canal. There was no fistulla or external leakage and the epithelial attach-ment was healthy and firm. Internally and inflammatory tissue was visible and bleeding occurred. The tissue was removed with a sharp excavator and the bleeding was stopped using a combination of 30% hydrogen peroxide, 5% sodium hypochlorite and dry cotton pellets. The perforation was visible now and it was about 3x2 mm large. MTA-mud was introduced in the root canal cavity in accordance with the projector technique. After 20 minutes the tooth was temporarily closed. At the second appointment after a few days the root canal therapy was completed. Controls took place after a week, a month, and 3 months. The tooth was without any symptoms. It is to conclude that MTA can be used also in conservative endodontic therapy of internal-external perforations.
