What happens before sickness presenteeism and sickness absenteeism in nurses occur? Results of a systematic review
Introduction: Healthcare is one of the most challenging industries for workforce. The work is complex, challenging, risky and responsibilities are immense It has become one of the most dangerous industries due to large numbers of occupational injuries and illnesses. These specific conditions are not only risky for nurses’ health but may trigger presenteeism and absenteeism as well. Sickness absenteeism is absence from work due to health. Sickness presenteeism is the opposite - people attend work despite being sick. Both behaviours represent challenge to nursing departments because they can increase costs, cause health care adverse events, and impact the quality of health care. Aims: This study comprehensively analysed and systemized elements associated with nursing sickness presenteeism (SP) and sickness absenteeism (SA). Methods: Systematic review of cohort studies was designed to be consistent with PRISMA guidelines. PubMed, ProQuest, and Emerald were systematically searched for peer reviewed articles published from the 1950s to December 2016. Cohort studies were included (12 SA and 1 SP) in the review if they examined the association between one or more exposures and SP and/or SA in nurses. The methodological quality of the included studies was assessed using the Newcastle‐Ottawa Scale. Results: 23 antecedents were associated with SA and grouped as work and organizational, mental and physical health, and demographic ; 3 antecedents were associated with SP (job demands, burnout, and exhaustion). Exhaustion (fatigue) and job demands were associated with SA and SP. Depersonalization was an outcome of SP over time. Discussion: The ability to predict SP and SA in nursing is useful to constrain costs and ensure that quality care is delivered. The antecedents to SA have shown to be numerous (work and organizational, mental and physical health, and demographic) and complex and mediating factors have been found. Conclusion: Hospitals should provide influenza vaccination before the onset of influenza season. Sick nurses should be allowed to take reasonable amounts of paid sick leave. Special attention should be placed at both fatigue/exhaustion and work demands since the both predictors were associated with SP and SA, working shorter hours, adequate number of nurses and not overloading with tasks.
