Prevalence and Clinical Relevance of Occult HCV Infection (OCI) - from a Pathologist's View
Occult HCV Infection (OCI) is a new form of CHC and was introduced by Castillo et al in 2004. OCI is defined as the presence of HCV-RNA in hepatocytes and the absence of HCV in the serum according to usual tests. OCI has been reported in several high-risk groups, especially in hemodialysis patients and subjects with cryptogenic liver disease. Currently, the detection of HCV-RNA in liver tissue (liver biopsies) or in peripheral blood mononuclear cells (PBMCs) occurring in an individual with undetectable HCV-RNA in serum represents the gold standard for the identification of an OCI. Pathohistological evidence of OCI is not convincing, and HCV-RNA identification is not routinely performed. RT-PCR detection of the HCV-RNA in liver biopsy specimens is expensive, unstandardized, and time-consuming. The alternative diagnostics tests should be applied in future.
