Video EEG polysomnography
Sleep medicine is a rapidly advancing, multidisciplinary field with strong links to clinical neurophysiology and clinical neurology. While some sleep disorders are diagnosed by history alone, others such as sleep apnoea, narcolepsy, periodic limb movement disorder and nocturnal seizures require evaluation in the sleep laboratory. Video-polysomnography with expanded electroencephalography (VEEG PSG) combines expanded EEG and PSG and high-quality video to evaluate complex nocturnal behaviours and paroxysmal motor activity when epileptic seizures are suspected. Advantages of VEEG PSG over routine PSG are: higher likelihood of recognizing interictal and ictal EEG activity, more precise evaluation of EEG background, and better correlation with clinical and neurophysiologic parameters. Since sleep disorders and epilepsy are common conditions in general population, co-morbidity and mutual influence is likely to exist. Sleep can activate the occurrence of seizures and epileptiform EEG abnormalities. In general, seizures and interictal epileptic discharges (IEDs) are more facilitated during non-rapid-eye- movement (NREM) sleep and suppressed during rapid-eye- movement (REM) sleep. It is thought that focal epileptiform EEG discharges in NREM sleep are facilitated by the synchronized EEG pattern. In contrast, during REM sleep the EEG pattern is desynchronized and thus less likely to propagate these discharges. The occurrence of real seizures during the sleep-wake cycle mainly depends on the seizure type and etiology. We present several patents where VEEG PSG helped in correct evaluation in patients with epilepsy and co-morbidity of other sleep disorders like obstructive sleep apnoea and parasomnias.
