Many patients scheduled for ambulatory surgery receive chronic anticoagulant therapy to minimize their long-term risk of thromboembolic disease. Increasing number of these patients and increasing number of outpatient procedures face anesthetists with a challenge to balance the risks of thromboembolism and bleeding.The bridging anticoagulant therapy is the administration of a short-acting parenteral anticoagulant agents during the peri-operative period, when the patient is not taking chronic oral anticoagulant. Bridging decision may be individualized in outpatients depending on individual thromboembolic risk, surgical bleeding risk which is specific for different procedures and anesthesia technique applied. Periprocedural care of patients on antithrombotic therapy especially those on DOACs represents a great challenge in regional anesthesia for one-day surgery. Antithrombotic agents may increase a risk of perioperative bleeding associated with regional anesthetic techniques and eye surgical procedures. Hemorrhagic complications may lead to the loss of vision or even loss of the eye. On the other hand, stopping antithrombotic agents before the surgery may put the patients to a higher risk of thromboembolic events with potentially life-threatening consequences. Increasing number of outpatients undergoing eye surgery are elderly who are usually on antithrombotic therapy. These drugs may increase a risk of perioperative bleeding associated with regional anesthetic techniques and eye surgery. Updated recommendations for continuing or stopping or modification antithrombotic therapy are needed in order to optimize management of antithrombotic therapies in outpatients scheduled for the eye surgery under ophthalmic regional blocks.
